But briefly? I’ll try. There’s a lot of answers, though.
Sam is Dean’s heart because a four year old boy in Kansas looked at his baby brother and decided he was going to fight and be brave. Sam Winchester became the pulse for almost every decision Dean would ever make.
Sam is Dean’s heart because Dean pushes aside anything good that anyone believes about him, and Sam picks it up and carries it for the day Dean will need it.
Sam is Dean’s heart because, when Dean thinks about laying down and giving up, a look from Sam will hit him in a rush. He sees Sam as an illuminated, time-lapse miracle. Sam at age 4, Sam at age 8, at age 12, at age 16… milestones in Dean’s head that all converge on Dean chaotically and swirl around him. The things most people forget, Dean cannot. He looks at Sam and he remembers it all, from the baby who grinned at him out of the crib 30 years ago to the man who smiled at him from across the car the day before.
If someone found a way to transfer that love out of Dean, the weight of it would kill them, without mercy or hesitation. But Sam is strong enough to carry it, and he does.
Sam is Dean’s heart because living without Sam is out of the question.

Reason number 2:
The Brothers.
The Winchester Brothers are, pretty much, the central reason I watch Supernatural, for me it’s all about Sam and Dean, all about their relationship and their journey, but mostly their relationship. What is it about them that has created such a level of devotion in me and so many people I know? Why do we fret and worry about them so? Why do we get so angry with them, why do they break our hearts and make us cry with such regularity? We get frightened for them, we fear for their safety, their sanity, we want to hold them, comfort them, help them. Sure they’re a handsome pair and sure we’re forever yelling things at the TV about Dean being hot or Sam being, well hot, but what is it really that keeps us with them week to week? What is it that makes so many of us need to talk about and feel connected to this pair of, dare I say it, fictional characters? Why do we love them so? I guess I can only speak for myself, and I’m not sure I’m going to be able to put it into words, but for me, it’s the depth of their relationship, the bond that they share, the fact that above and beyond everything else, no matter what they’re faced with they’ll always fight for each other. Sure, there have been times when they’ve been separated, even when it’s just been a psychological separation, there’s been times when there’s been a gulf between them, but you know that when push comes to shove they will always stand by each other. We’ve seen them do it time and again. With everything that has happened to them, all the tragedy they have faced, they keep moving forward, they keep fighting, they never give up and they always stick together. They won’t let evil win, they won’t let it beat them down and it never will because they’ll always have each other. Their relationship may appear disfunctional to some, but I don’t know, to have a connection that deep, to have someone that dedicated to you, to know that one person will always, no matter what, have your back and do anything for you, that sounds pretty functional to me. Their love is never professed but they say it in subtle and not so subtle ways, you know like dying for each other, but most of the time it’s just a little look, a wink, a smile or a nod, they don’t need to put it into words because you can just feel it, their love for each other rolls off them. They don’t always agree, they disappoint each other, they’re stubborn and they fight and bitch like all good siblings but when they need to, they put all that behind them and then they’re a force to be reckoned with. They may be each other’s weakness but they’re also each other’s touchstone, each other’s moral compass, they’re soul mates and neither will ever be truly whole with out the other. Sure they’re brave and strong, good fighters, good friends, funny, loyal and caring and everything that makes them wonderful individuals but it’s the two of them together that makes them extraordinary. It’s the brothers, it’s Sam and Dean and you what, I love them like they’re real.
